ABSTRACT

Objective:To determine the expression of Sprouty in skin lesions of patients with psoriasis vulgaris, and to investigate its correlation with the severity of psoriasis.Methods:From May 2018 to November 2018, 15 lesional skin samples and 10 normal skin samples were collected from patients with psoriasis vulgaris and healthy controls respectively in Dermatology Hospital of Southern Medical University. Immunohistochemical study was performed to determine the distribution of Sprouty1, 2 and 4 proteins in psoriatic lesions and normal skin tissues, Western blot analysis to determine the protein expression of Sprouty1, 2 and 4, reverse transcription (RT) -PCR to measure the mRNA expression of Sprouty1, 2 and 4. Pearson correlation analysis was used to analyze the correlation between Sprouty protein expression and psoriasis area and severity index (PASI) scores.Results:Immunohistochemical study showed that Sprouty1, 2 and 4 were all expressed in psoriatic lesions and normal skin tissues. Sprouty1 was strongly expressed in the cell membrane and cytoplasm of granular layer cells in the control group, but slightly expressed in the cell membrane of granular layer cells in the psoriasis group. Sprouty4 was more strongly expressed in the psoriasis group than in the control group, and Sprouty2 was slightly expressed in both the 2 groups. RT-PCR and Western blot analysis showed that Sprouty1 mRNA and protein were both expressed in the psoriasis group and control group, and significantly lower in the psoriasis group (0.844 ± 0.169, 0.148 ± 0.141 respectively) than in the control group (0.972 ± 0.105, 0.413 ± 0.108 respectively, both P < 0.05) ; the Sprouty1 protein expression was negatively correlated with the PASI score in the psoriasis group ( r = -0.628, P = 0.012) . The psoriasis group showed significantly increased protein and mRNA expression of Sprouty4 (1.306 ± 0.283, 1.727 ± 1.017 respectively) compared with the control group (0.584 ± 0.304, 0.714 ± 0.615, t = 6.063, 2.814, respectively, both P < 0.05) , and the Sprouty4 protein expression was positively correlated with the PASI score in the psoriasis group ( r = 0.812, P < 0.001) . The Sprouty2 protein and mRNA were slightly expressed in the 2 groups, and no significant difference was observed between the 2 groups (both P > 0.05) ; there was no significant correlation between the Sprouty2 protein expression and PASI score ( r = 0.436, P = 0.104) . Conclusion:The protein expression of Sprouty1 and 4 in the psoriatic lesions is correlated with the PASI score, suggesting that Sprouty1 and 4 are related to the occurrence of psoriasis.

In order to understand the educational organization status of doctor-patient communication courses, 114 medical colleges and universities of China were investigated, including course offerings, teaching methods and teacher condition. The survey showed that the ratio of offering doctor-patient communication courses was 64.9%, but only 51.3% courses were compulsory. The setting of class hours was arbitrary. The target objects were mainly concentrated on medical students before entering clinical practice and the large-class teaching was a main mode. The average number of teachers was three to four, half of whom were part-time teachers. Therefore, we suggest attaching great importance to doctor-patient communication course by setting it as a compulsory course, emphasizing the practicability of the course in teaching content, and properly handling the relationship between full-time teachers and part-time teachers.

ABSTRACT

Objective To study the risk factors for postoperative nausea and vomiting (PONV) in gynecological patients administered prophylactic antiemetics.Methods A total of 1 000 gynecological surgical cases were reviewed.Data on patient characteristics,anesthesia,surgeries,and 1st postoperative day nausea and vomiting were collected and analyzed statistically.Results Multivariate logistic regression analysis included a history of motion sickness,the use of an analgesic pump,the duration of anesthesia,and the use of neostigmine.The odds ratios (ORs),in decreasing order,were that of a history of motion sickness [OR =1.953,95% confidence interval (CI):1.404-2.716],the use of neostigmine (OR =1.831,95% CI:1.203-2.786),the use of an analgesic pump (OR =1.472,95% CI:1.017-2.129),and the duration of anesthesia (OR =1.256,95% CI:1.058-1.492).The predictive model was described by the following equation:P=1/(1+e(1623-0.669X1-0.386X2-0.228X3+0.605X4)).Tne predictive accuracy of this model for the incidence of PONV in gynecological patients was 70.6%,and the area under the receiver operating characteristic curve for the predictive result was 0.642.Conclusion The independent risk factors for PONV in gynecological patients administered prophylactic antiemetics are a history of motion sickness,the use of neostigmine,the use of an analgesic pump,and the duration of anesthesia,in decreasing order of significance.

ABSTRACT

Objective To analyze and discuss the clinical effect of sequential radiotherapy and chemotherapy in 40 patients with advanced gastric cancer after operation.Methods 80 postoperative gastric cancer patients who received sequential radiotherapy and chemotherapy in our hospital were prospectively analyzed.According to the order of admission,80 patients were divided into two groups,40 cases of the control group(chemotherapy)and 40 cases of the observation group(sequential chemotherapy).The clinical effects of the two groups were compared.Results In the observation group,1 year survival rate and 2-year survival rate(77.50%,47.50%)were higher than those in the control group(50.00%,25.00%),there were statistically significant differences between the two groups(x2=19.20,32.65,all P<0.05).The incidence rate of adverse reactions of the control group was 12.50%,which was lower than 30.00% of the observation group,there was statistically significant difference between the two groups(x2=9.15,P<0.05).The mortality rate of the observation group was 32.50%,which was lower than 45.00% of the control group,the difference was statistically significant(x2=5.29,P<0.05).Conclusion Compared with the simple chemotherapy after the operation of gastric cancer,sequential radiotherapy and chemotherapy can effectively improve the clinical efficacy,significantly reduce the rate of postoperative metastasis and recurrence of gastric cancer,and it is worthy of promoting.

ABSTRACT

Objective To study the analysis of non-small cell lung cancer patients with multiple brain metastases with erlotinib combined with whole brain radiation therapy clinical observation and effect of C on vascular endothelial growth factor level. Methods 40 cases of non - small cell lung cancer patients with multiple brain metastases treated in Taizhou tumor hospital from January 2015 to April 2016 were selected and randomly divided into the control group and the experimental group, with 20 patients in each group. The control group and the experimental group patients were given clinical treatment of whole brain radiotherapy, the control group was given routine treatment, the experimental group received erlotinib. The clinical effects of the 2 groups were compared and analyzed. Results After the corresponding treatment, the experimental group of 20 patients, 8 cases of complete remission, 7 cases of partial remission, the number of effective treatment for 15 cases, the treatment rate was 75.0%. Of the patients in the control group, 6patients had complete remission, and 4 patients had partial remission. The effective rate was 50%. Available, the effective rate of the treatment group (75.0%) was significantly higher than that of the control group (50.0%), with statistical difference (P<0.05). The survival rate of the experimental group after one year (80.0%) was significantly higher than that of the control group (60.0%), with statistical difference (P<0.05). The level of vascular endothelial growth factor (C) in the experimental group was significantly lower than that in the control group (P<0.05). Conclusion Non small cell lung cancer patients with multiple brain metastases with erlotinib combined with radiotherapy in the clinical treatment effect of whole brain is better, can improve the survival rate in a large extent, improve the endothelial growth factor C levels, with the further promotion of the clinical significance.

ABSTRACT

Objective:To investigate the efficacy and safety of brain protein hydrolysate injection in the treatment of elderly patients with parkinson.Methods:120 patients with parkinson were selected and randomly divided into two groups.The control group (57 cases) was treated by routine treatment,while the observation group (63 cases) was given brain protein hydrolysate injection on the basis of routine treatment.The UPDRS scores,SF-36 scores and adverse reactions during treatment were observed and recorded.Results:Before treatment,the UPDRS and SF-36 scores of both groups had no significant difference (P>0.05).After treatment,the UPDRS scores of both groups were significantly decreased,and the UPDRS score of observation group was lower than that of the control group(P<0.05).The social function,emotional function,mental health score of observation group were higher than those of the control group(P<0.05).During treatment,1 cases of dizziness,1 cases of nausea were found in the control group.The adverse reaction rate was 3.5%.2 cases of insomnia,1 cases of fatigue,1 cases of dizziness and 1 cases of nausea were found in the observation group.The adverse reaction rate was 7.9%.There was no statistical difference in the incidence of adverse reactions between two groups (P>0.05).Conclusion:Brain protein hydrolysate injection had significant effect on the Parkinson.It could improve the patients' thinking ability,mental health and quality of life with high safety.

ABSTRACT

Objective To evaluate and compare the effectiveness of propofol-remifentanil closed-loop and open-loop anesthesia in gynecological laparoscopic operation under bispectral index (BIS) monitoring.Methods Forty female patients undergoing elective gynecological laparoscopic operation were recruited and randomly divided into closed-loop (group Ⅰ) and open-loop (group Ⅱ) groups.During anesthesia maintenance,the closed-loop group was administered with a BIS-feedback system to regulate the target effect-site concentration;whereas,the open-loop group was administered the target effect-site concentration according BIS value manually.The variation of non-invasive mean arterial pressure (MAP),heart rate (HR),SpO2,BIS,extubation time,consumption of propofol and remifentanil,Ramsay index,and subjective comfort grade were recorded at the selected time points.Results The extubation time in group Ⅰ was shorter than in group Ⅱ.The total dosage of propofol administered in group Ⅰ was less than that in group Ⅱ,but there was no significant difference in the total consumption of remifentanil.HR in group Ⅰ was steadier than in group Ⅱ.However,the MAP and Ramsay index were similar in both the groups.The subjective comfort grade in group Ⅰ was higher than in group Ⅱ.Conclusion The use of propofol-remifentanil closed-loop system by BIS-feedback anesthesia is safer,more controllable,with higher degree of satisfaction and sparing side-effects,we therefore recommend it during gynecological laparoscopic operations.

Objective To recognize the existing problems of master of nursing specialist students during clinical practice.Methods Eight master of nursing specialist students from 4 general hospitals in Changchun city were selected as respondents.Data were collected by focus group interview and analyzed by qualitative research method.Results Three formed themes included feeling of great pressure in clinical practice,existing problem in clinical practice teaching and assessment.Conclusions School and hospital need cooperate to carry out a training on competence of nurses,establish a comprehensive supervised clinical practice teaching and a assessment system with nursing features.

ABSTRACT

Objective To investigate the conditions of diabetes-related knowledge as well as the influencing factors in elderly people in Changchun so as to provide basis for regulation of health education policy.Method A total of 400 community elderly people in Changchun involved in the survey by questionnaire for investigating the knowledge on diabetes and the influencing factors. Results The elderly people in the community in Chuangchun knew much better about the typical symptoms,diet and treatment by 74.0%,80.8%and 75.8%,respectively,but they knew little about the causes of the disease and complications by 52.7%and 25.7%respectively.The factors influencing their acquisition of diabetes related knowledge included education level,income and disease condition.Conclusions The elderly people in the communities lack diabetic knowledge,which is unbeneficial for the prevention and control of diabetes.Therefore,it is a need to strengthen diabetes-related knowledge education in order to prevent and reduce the complications.

ABSTRACT

Objective To study the expression and regulation characteristics of aquaporin water channel(AQP) of cerebuller metastatic tumor and brain tissue surrounding tumor and to understand the role of abnormal expression of AQP in formation and elimination of brain edema.Methods The tumor tissue specimens from five lung adencarcinoma brain metastasis cases after surgical resection were accessed.The total RNA was extracted,and RT-PCR,immunohistochemical staining were progressed to study the expression and regulation characteristics of AQP.The average gray values of near and far regions from tumor tissue were measured with HPIAS-1000 cells measuring procedure to compare actual expression quantity of AQP.Results AQP4 had a high expression in the peritumoral brain tissue and no expression in the center of brain metastasis tumor organization.The AQP4 staining was junior in the more distant region from tumor and it added significantly in the region close to the tumor tissue.Stained AQP1 was not found in cerebullar metastatic tumor and peritumoral brain microvascular endothelial cells.Conclusion The expression of AQP4 is increased significantly in the region next to the cerebullar metastatic tumor tightly.It is probably related to the formation of peritumoral brain edema.The negative expression of AQP1 might not be conducive to the removal of edema in the interspace of nerve cells.

ABSTRACT

OBJECTIVE To observe the curative effect of continuous lumbar cisterna drainage for severe intracranial infection. METHODS We cured 24 patients of severe intracranial infection with the method of continuous lumbar cisterna drainage and injecting sensitive antibiotics to subarachnoid space. RESULTS Making use of continuous lumbar cisterna drainage and injecting sensitive antibiotics to subarachnoid space turned to be an effective method for severe intracranial infection,all patients had been cured. CONCLUSIONS In patients of severe intracranial infection,the responsible agents have been transformed,we conclude that it improves the outcome obviously to drain the CSF and inject sensitive antibiotics to subarachnoid space.

ABSTRACT

Aim To explore the effects of heat stress on TNF-?-induced fever and the content of cAMP in rat hypothalamus. Methods ①We examined the peak time of HSP70 expression in hypothalamus after heat stress; ②We studied the change of TNF-?-induced fever and the content of cAMP in hypothalamus after heat stress. Result The maximum expression of HSP70 in hypothalamus was attained at 12 h after heat stress. Heat stress limited TNF-?-induced fever and decreased the content of cAMP in hypothalamus. Conclusion Heat stress can inhibit TNF-?-induced fever in rats, and this effect may be related with the inhibition of cAMP synthesis induced by high expression HSP70 in hypothalamus.
